That is why I love these simple questionnaires. You ask your kids a few questions about Dad or Grandpa and write down what they say. Their answers might be sweet, funny, or a little bit of both!

I have done this activity with my own kids for Mom, Grandma, Dad and Grandpa. It is always fun to hear how kids see the special people in their lives.

You don’t need an expensive gift or a complicated craft to make Father’s Day special. These one-page printables are a simple, inexpensive way for kids to give Dad and Grandpa something personal.

How to Use These Father’s Day Questionnaires

All you need is a printed questionnaire, a pencil and a few minutes with your child.

Read each question aloud and write down your child’s answer. Older kids can fill in their own answers, while younger children can tell you what to write.

My favorite part? Hearing the answers they come up with all on their own!

Try to write down exactly what your child says. It can be tempting to help them come up with an answer, but their own words are what make this gift special. Even if their guess about Dad’s age is nowhere close!

If you have more than one child, print a separate page for each one. It is fun to see how differently they answer the same questions about the same person.

When the questions are finished, let your child color the border or draw a picture on the back. Add their name and the date so you can remember how old they were when they completed it.

You can tuck the finished page inside a Father’s Day card, give it with another gift, or let it be the gift all on its own.
